Licensed registered nurse to provide direct nursing care and case management for clients in their homes and other settings in Laramie County.  Must have current Wyoming RN License, valid Wyoming Driver’s License in good standing and must pass background check.  Preference given with experience in home health.  Full time benefited position working Mon-Fri, 8 am to 5 pm. Certified Nurse Assistant

Closing Date: When position is filled                    Position:  PRN                  Salary Range:  $11.00-$13.00\hr

General Description of work: Under supervision of the client’s case manager and other public health nurse managers to perform a variety of duties involved in providing personal care to patients in their homes; and to assist in nursing clinics.

Essential Functions: Provide personal care to patients in their homes.  Perform simple household tasks in patient’s homes including dusting, bed-making, laundry, meal preparation, and shopping.  Observe patients for changes in condition; report changes to nurse; document care given in patient records according to standards for documentation; prepare reports of activities.  Assist in clinics including answering phones; maintaining records; greeting and routing patients; setting up and cleaning exam rooms.  Travel via automobile throughout Laramie County and occasionally outside Laramie County to patient’s homes, clinics and meetings.  Write and submit statistical and narrative reports of activities including Public Health Nurse Informatics (PHNI).  Perform other duties as assigned by the supervising nurse or county manager.  Respond to public health emergencies.

Knowledge of: Modern office practices and methods.   Principles and procedures of record keeping and reporting.  Principles and practices used in dealing with the public.  Principles of normal and abnormal skin care.  Basic principles of ageing and disability.  Basic principles of infection control.  Safe driving principles and practices.

Skill to: Operate lifts, wheelchairs, and other assistive equipment.  Operate a variety of medical clinic equipment and devices.  Operate modern office equipment with basic computer skills.  Operate a motor vehicle safely. 

Ability to: Perform responsible patient care.  Observe patient status; report on patient status to supervising nurse.  Prepare and maintain accurate, complete, clear and concise records.  Interact effectively and sensitively with individuals from diverse backgrounds.  Act quickly and calmly in emergencies.  Maintain confidentiality of patient information.  Communicate clearly both orally and in writing. 

Experience and Training Guidelines: Any combination equivalent to experience and training that would provide the required knowledge, skills and abilities would be qualifying.  Some nursing assistant experience is desirable.  Home health nursing assistant experience is desirable.  Equivalent to the completion of the twelfth grade supplemented by completion of a nursing assistant course.

License or Certification: Certification as a Nursing Assistant in the State of Wyoming. Possession of an appropriate valid driver’s license.  Receive a Home Health Endorsement from the Wyoming State Board of Nursing two weeks after hire date.

Special Requirements: Essential duties require the following physical skills and work environment: Ability to sit, stand, walk, kneel, stoop, and lift 50 pounds; exposure to biological hazards; ability to travel to different sites and locations; availability for shift work, on-call, and stand-by.
